In medicine we have government without newspapers. Patients
cannot find out what they need to know to make informed choices. No one in
medicine records or reports the information patients need to know the most. So
patients will have to.

For instance
For instance, a good guide for any complaint are these two instructions from the
Wyoming Board of Medicine's complaint form:
12. Please describe in as much detail as possible the nature of the illness
or condition for which the Physician and/or Physician Assistant were utilized.
Please attached additional pages, records, documents, etc, as necessary.
13. Please describe in as much detail as possible the
facts and circumstances about which you are complaining. Also attach additional
pages as well as any documentation, patient charts, etc. that may have bearing
on this matter. Please be sure to include any efforts to resolve this matter
prior to bringing it to the Board's attention.
In Wyoming they also require that you enclose a
release of medical information form
on which you signature has been notarized.
